To request a mobility scooter permit online, please complete the below form along with any evidence required and click submit.
Any information provided must be accurate to the mobility scooter intended for use with your permit. If details submitted are not accurate, this may result in your permit being withdrawn for safety reasons. This will then be processed by our Customer Services Team who will get in touch with you.
Please be aware that successful permit applications may take up to 10 working days to process & arrive with you by post.
